В последние годы для лечения пациентов с грыжами спигелиевой линии (ГСЛ) применяются различные виды открытых, эндовидеохирургических оперативных вмешательств и разнообразные варианты размещения сетчатых эндопротезов. В связи с редкой встречаемостью данной патологии, недостаточной освещенностью этой проблемы в мировой литературе требуется разработка оптимальной хирургической тактики у пациентов с ГСЛ.</p></sec><sec><title>Цель</title><p>Цель. Обосновать тактику хирургического лечения больных с ГСЛ на основании сравнительного анализа непосредственных, отдаленных, косметических результатов различных видов оперативных вмешательств.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено сравнительное изучение непосредственных и отдаленных результатов оперативного лечения 69 пациентов с ГСЛ. Из них в 1-ю группу включили 22 (31,9%) пациента, которым выполнили открытую натяжную герниопластику, 2-ю группу составили 24 (34,8%) больных, перенесших ненатяжную герниопластику из традиционного доступа, в 3-ю группу вошли 15 (21,7%) пациентов после лапароскопической интраабдоминальной герниопластики, 4-я группа была составлена из 8 (11,6%) пациентов, перенесших экстраперитонеальную видеэндоскопическую герниопластику.</p></sec><sec><title>Результаты</title><p>Результаты. Средняя продолжительность открытых операций у пациентов 1-й и 2-й групп была в 1,8–2,2 раза меньше, чем длительность эндовидеохирургических вмешательств у пациентов 3-й и 4-й групп. Выраженность послеоперационной боли по ВАШ после открытых операций была в 1,6–2,1 раза больше, чем у пациентов, перенесших эндовидеохирургическую герниопластику. В раннем послеоперационном периоде раневые осложнения отмечены у 1 (4,2%) пациентки 1-й группы и у 1 (4,5%) больной 2-й группы, у пациентов 3-й, 4-й групп осложнений не наблюдали. Рецидив грыжи выявлен в отдаленном периоде только у 1 (4,8%) больного 1-й группы после открытой натяжной герниопластики.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Применение эндовидеохирургических вмешательств позволяет снизить травматичность операции, уровень послеоперационной боли и послеоперационных осложнений, улучшить косметический результат операции.</p></sec><sec><title>Заключение</title><p>Заключение. Наиболее эффективными операциями у пациентов с ГСЛ являются открытая и эндовидеохирургическая интраабдоминальная, экстраперитонеальная протезирующие герниопластики.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In recent years, various types of open, endovideosurgical surgical interventions and various options for the placement of mesh endoprostheses have been used to treat patients with spigel line hernias (GSL). Due to the rare occurrence of this pathology and the insufficient coverage of this problem in the worldliterature, it is necessary to develop optimal surgical tactics in patients with HSV. Aim. To substantiate the tactics of surgical treatment of patients with HSV based on a comparative analysis of the immediate,long-term, and cosmetic results of various types of surgical interventions.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comparative study of the immediate and long-term results of surgical treatment of 69 patients with HSV was conducted. Of these, group 1 included 22 (31.9%) patients who underwent open tension hernioplasty, group 2 included 24 (34.8%) patients who underwent non-tension hernioplasty from traditional access, group 3 included 15 (21.7%) patients afterlaparoscopic intra-abdominal hernioplasty, group 4 consisted of 8 (11.6%) patients who have undergone extraperitoneal videoendoscopic hernioplasty.</p></sec><sec><title>Results</title><p>Results. The average duration of open surgeries in patients of groups 1 and 2 was 1.8–2.2 timesless than the duration of endovideosurgical interventions in patients of groups 3.4. The severity of postoperative pain on the VAS scale after open surgeries was 1.6–2.1 times greater than in patients who underwent endovideosurgical hernioplasty. In the early postoperative period, wound complications were noted in 1 (4.2%) patient of the first group and in 1 (4.5%) patient of the second group, no complications were observed in patients of 3.4 groups. Hernia recurrence was detected in thelong-term period in only 1 (4.8%) patient of the first group after open tension hernioplasty.</p></sec><sec><title>Discussion</title><p>Discussion. The use of endovideosurgical interventions can reduce the traumatic nature of surgery, the level of postoperative pain and postoperative complications, and improve the cosmetic result of surgery.</p></sec><sec><title>Conclusion</title><p>Conclusion.
