В последние годы эндовенозная лазерная коагуляция (ЭВЛК) стала основным методом лечения варикозной болезни нижних конечностей, однако отдаленные клинические результаты ЭВЛК и классического хирургического вмешательства схожи, а анатомические причины рецидива при дуплексном сканировании различны.</p></sec><sec><title>Цель</title><p>Цель. Сопоставить частоту повторного возникновения варикозной болезни после проведения изолированной ЭВЛК большой подкожной вены (БПВ) и комбинированной процедуры, включающей переднюю добавочную вену, в отдаленном послеоперационном периоде.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено сравнительное исследование, в котором приняли участие 162 человека с варикозным расширением вен на ногах. Всем им была проведена эндовазальная лазерная коагуляция вен в области БПВ. Пациенты были разделены на две группы: группа I – 81 человек, которым была проведена изолированная ЭВЛК БПВ; группа II – 81 человек, которым была проведена комбинированная ЭВЛК БПВ и передней добавочной вены (ПДБПВ).</p></sec><sec><title>Результаты</title><p>Результаты. При анализе ранних послеоперационных осложнений в группах исследования статистически значимых различий не было установлено. По результатам контрольного ультразвукового дуплексного сканирования, проведенного через три года после операции, было обнаружено: в группе I (ЭВЛК БПВ) частота рефлюкса в области бедра статистически значимо выше – 15 (18,5%), чем в группе II (ЭВЛК БПВ + ПДБПВ) – 1 (1,2%). Разница статистически значима (р &lt; 0,05). Обсуждение. Исследования показывают, что профилактическая ЭВЛК передней добавочной вены значительно снижает частоту рецидивов варикозной болезни, особенно при наличии рефлюкса в этой вене.</p></sec><sec><title>Выводы</title><p>Выводы. Облитерация добавочных подкожных вен вне зависимости от наличия рефлюкса и их размера является оправданной тактикой для снижения количества рецидивов в отдаленном послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. In recent years, endovenous laser coagulation (EVLC) has become the primary method for treating varicose vein disease of the lower extremities. However, the long-term clinical outcomes of EVLC are similar to those of classical surgical interventions, while the anatomical causes of recurrence identified through duplex scanning differ.</p></sec><sec><title>Aim</title><p>Aim. To compare the recurrence rates of varicose vein disease after isolated EVLC of the great saphenous vein (GSV) and a combined procedure that includes the anterior accessory vein (AASV) in the long-term postoperative period.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comparative study was conducted involving 162 individuals with varicose veins in the legs. All participants underwent EVLC of the GSV. The patients were divided into two groups: Group I consisted of 81 individuals who underwent isolated EVLC of the GSV; Group II included 81 individuals who underwent combined EVLC of the GSV and AASV.</p></sec><sec><title>Results</title><p>Results. Early postoperative complications revealed no statistically significant differences between the groups. According to the results of follow-up ultrasound duplex scanning performed three years after the surgery in Group I (EVLC of GSV), the reflux rate in the thigh area was statistically significantly higher at 15 (18.5%) compared to Group II 1 (1.2%) (EVLC of GSV + AA SV). This difference was statistically significant (p &lt; 0.05).</p></sec><sec><title>Discussion</title><p>Discussion. The studies show that preventive EVLC of the AASV significantly reduces the recurrence rates of varicose vein disease by 25%. Conclusions.
