Введение. В амбулаторной колопроктологии при подготовке кишечника к инструментальному исследованию большое значение придается эффективному лечению запора и устранению копростаза с использованием современных слабительных средств.

Цель

Цель. Оценить клиническую эффективность применения комбинированного ректального слабительного средства в форме раствора для устранения копростаза, обусловленного запором, и подготовки дистальных отделов кишечника к инструментальному исследованию у колопроктологических больных в амбулаторных условиях.

Материалы и методы

Материалы и методы. В исследование включены 135 пациентов колопроктологического профиля с проявлениями функционального запора и копростаза, которые в зависимости от варианта подготовки толстого кишечника к инструментальному исследованию (видеоректороманоскопия, видеоассистированная аноскопия) были разделены на три группы. У 41 пациента применялись пероральные слабительные на эмпирической основе (1-я группа); 49 пациентам выполнялись стандартные очистительные клизмы (2-я группа); 45 пациентам применялось комбинированное ректальное слабительное средство (3-я группа). Качество подготовки дистальных отделов кишечника к инструментальному исследованию оценивали на основе унифицированной Бостонской шкалы. В последующем в рамках исследуемых групп у 60 пациентов с различными колопроктологическими заболеваниями доброкачественного генеза, выявленными в ходе обследования, продолжено комплексное лечение запора с использованием указанных методов.

Результаты и обсуждение

Результаты и обсуждение. Наилучшие результаты были получены при подготовке кишечника с помощью комбинированного ректального слабительного средства (3-я группа пациентов). В этом случае количество пациентов с отличной и хорошей подготовкой составило 95,5%. На этапе последующего лечения хронического запора у колопроктологических больных в условиях применения данного слабительного средства у 82,8% пациентов отмечалось формирование размягченного кала (тип 3 и 4) или кашицеобразного стула (тип 5 и 6). Это превышало аналогичные результаты, полученные при лечении запора пероральными слабительными препаратами (39,3%) или посредством очистительных клизм (74,7%).

Заключение

Заключение. Применение комбинированного ректального слабительного средства в амбулаторной колопроктологии позволяет максимально быстро ликвидировать копростаз и в 95,5% случаев качественно подготовить дистальные отделы кишечника к инструментальному исследованию. Применение исследуемого средства в комплексном лечении хронического запора у колопроктологических больных способствует восстановлению регулярного стула в 90% случаев.

Introduction. In outpatient coloproctology, when preparing the intestine for instrumental examination, great importance is attached to the effective treatment of constipation and elimination of coprostasis.

Aim

Aim. To evaluate the clinical efficacy of the rectal laxative to eliminate coprostasis caused by constipation and prepare the distal intestine for instrumental examination in coloproctological patients on an outpatient basis.

Materials and methods

Materials and methods. The study included 135 patients with manifestations of functional constipation and coprostasis, who were divided into three groups. Oral laxatives were used empirically in 41 patients (group 1). Standard cleansing enemas were performed in 49 patients (group 2). The combined rectal laxative (group 3) was used in 45 patients. The quality of training was assessed based on the unified Boston scale. Subsequently, within the study groups, in 60 patients with various proctological diseases of benign genesis, identified during the examination, complex treatment of constipation was continued using the above methods.

Results and discussion. The best results were obtained when preparing the intestines with the rectal laxative (group 3 patients). In this case, the number of patients with excellent and good training was 95.5%. At the stage of subsequent treatment of chronic constipation in coloproctological patients with laxative Lax Canon, 82.8% of patients showed the formation of softened feces (type 3 and 4) or mushy stools (type 5 and 6). This exceeded similar results obtained in the treatment of constipation with oral laxatives (39.3%) or through cleansing enemas (74.7%).

Conclusion

Conclusion. The use of the rectal laxative in outpatient coloproctology makes it possible to eliminate coprostasis as quickly as possible and in 95.5% of cases to qualitatively prepare the distal parts of the intestine for instrumental examination. The use of the studied agent in the complex treatment of chronic constipation in proctological patients helps restore regular bowel movements in 90% of cases.
