В настоящее время диагностика этих состояний остается сложной задачей из-за неспецифического характера клинических симптомов и ограничений существующих диагностических инструментов. Целью обзора является изучение потенциала интестинального белка, связывающего жирные кислоты (I-FABP), в качестве биомаркера нарушения мезентериального кровообращения и повреждения кишки. В статье рассмотрены структура и функция I-FABP, его экспрессия и распределение в тонкой кишке, механизмы высвобождения. Анализируются клинические исследования уровней I-FABP у пациентов с этими состояниями, оценивается точность диагностики и потенциальные пороговые значения. Результаты исследований показывают, что I-FABP обладает значительным потенциалом в качестве биомаркера острой мезентериальной ишемии, ишемии и повреждения кишки. Повышенные уровни I-FABP наблюдались у пациентов, а диагностическая точность I-FABP, по-видимому, превосходит другие используемые в настоящее время биомаркеры. Лабораторные тесты, такие как лактат и D-димер, применяются в диагностике острой мезентериальной ишемии и повреждения кишки, но их диагностическая точность ограничена низкой специфичностью и высоким уровнем ложноположительных результатов. I-FABP можно использовать для мониторинга активности заболевания и ответа на лечение при таких состояниях, как воспалительное заболевание кишечника и глютеновая болезнь, потенциально улучшая результаты лечения пациентов за счет оптимизированных терапевтических стратегий. Однако необходимы дальнейшие исследования для определения оптимальных пороговых значений и установления стандартизированных протоколов измерения I-FABP. Таким образом, внедрение I-FABP в качестве биомаркера в клиническую практику может привести к более ранней диагностике и улучшению исходов у пациентов с ишемией кишки.</p></abstract><trans-abstract xml:lang="en"><p>Violation of mesenteric circulation and intestinal damage are life-threatening conditions that require early diagnosis and, if necessary, prompt surgical intervention. Currently, the diagnosis of these conditions remains a difficult task due to the nonspecific nature of clinical symptoms andlimitations of existing diagnostic tools. The aim of the study is to study the potential of intestinal fatty acid binding protein (I-FABP) as a biomarker of mesenteric circulatory disorders and intestinal damage. The article discusses the structure and function of I-FABP, its expression and distribution in the small intestine, the mechanisms of release of I-FABP. Clinical studies of I-FABP levels in patients with these conditions are analyzed, assessing the accuracy of diagnosis and potential thresholds. The results of this study show that I-FABP has significant potential as a biomarker of OMI, intestinal ischemia and intestinal damage. Elevated levels of I-FABP have been observed in patients, and the diagnostic accuracy of I-FABP appears to be superior to other currently used biomarkers. However, further research is needed to determine optimal thresholds and establish standardized I-FABP measurement protocols.
