Варикозная болезнь нижних конечностей – одно из самых распространенных заболеваний, которым страдает от 20 до 40% населения. Одной из самых широко используемых методик лечения является эндовенозная лазерная коагуляция (ЭВЛК), которая выполняется амбулаторно, быстро и обеспечивает значимый эстетический и хирургический результат.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность применения местной терапии гелем на основе эсцина, гепарина и эссенциальных фосфолипидов в послеоперационном ведении пациентов после эндовенозной лазерной коагуляции с минифлебэктомией.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было пролечено 53 пациента с диагнозом «Варикозная болезнь нижних конечностей» классов С2-С3 по СЕАР. Пациенты были разделены на 2 группы. Всем больным проводилась эндовенозная лазерная облитерация ствола большой подкожной вены в сочетании с минифлебэктомией притоков на бедре, голени. В первой группе в первые 14 дней не использовались топические гели, во второй группе со следующего после операции дня применялось топическое средство с содержанием гепарина, эсцина и эссенциальных фосфолипидов (Детрагель). В предоперационном и раннем послеоперационном периоде оценивали значимость болевого, отечного синдрома, выраженность гематом и флебитов.</p></sec><sec><title>Результаты</title><p>Результаты. В обеих группах наибольшее количество составляли пациенты с несостоятельностью ствола большой подкожной вены (БПВ) до границы в/3 и с/3 голени с дальнейшим сбросом по притокам БПВ по медиальной поверхности голени. Несостоятельность ствола БПВ до уровня щели коленного сустава сопровождалась наличием варикозно-трансформированных притоков по передней поверхности бедра, в области коленного сустава. Добавление в местное лечение геля на основе эсцина, гепарина и эссенциальных фосфолипидов привело к купированию флебитов и послеоперационных гематом, выраженному уменьшению болевого и отечного синдрома.</p></sec><sec><title>Обсуждение</title><p>Обсуждение. Трансдермальная доставка лекарственных средств (ЛС) имеет большие перспективы как альтернатива пероральному и внутривенному лечению, особенно для пациентов, страдающих хроническими заболеваниями. Трёхкомпонентный состав позволяет воздействовать на все звенья патогенеза заболевания.</p></sec><sec><title>Выводы</title><p>Выводы. Полученные данные об использовании местной терапии гелем на основе эсцина, гепарина и эссенциальных фосфолипидов после проведения ЭВЛО БПВ в сочетании с минифлебэктомией свидетельствуют о необходимости применения местной терапии для улучшения качества жизни пациентов, их быстрой реабилитации и улучшения непосредственных и отдаленных результатов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>RETRACTED ARTICLE</title><p>RETRACTED ARTICLE</p></sec><sec><title>Introduction</title><p>Introduction. Varicose veins of the lower extremities are one of the most common diseases, affecting 20 to 40% of the population. One of the most common treatment methods is endovenous laser coagulation (EVLC).</p></sec><sec><title>Aim</title><p>Aim. To evaluate the effectiveness of local gel therapy based on escin, heparin and essential phospholipids in the postoperative administration of patients after endovenous laser coagulation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A complex treatment of 53 patients with varicose veins of the lower extremities of classes C2-C3 according to the CEAP nomenclature was carried out. Patients were divided into 2 statically homogeneous groups with an equal number. All patients underwent endovenous laser obliteration of the trunk of the great saphenous vein in combination with miniphlebectomy of tributaries on the thigh and lower leg.</p></sec><sec><title>Results</title><p>Results. In both groups, the largest number were patients with incompetence of the GSV trunk up to the border in /3 and c/3 of the lower leg, with further discharge along the tributaries of the GSV along the medial surface of the lower leg. The failure of the GSV trunk to the level of the knee joint gap was accompanied by the presence of varicose-transformed tributaries along the anterior surface of the thigh, in the area of the knee joint. The addition of Detragel led to the relief of phlebitis and hematomas, a pronounced decrease in pain and edematous syndrome.</p></sec><sec><title>Discussion</title><p>Discussion. Transdermal drug delivery has great promise as an alternative to oral and intravenous treatment, especially for patients suffering from chronic diseases. The three-component composition allows you to influence all parts of the pathogenesis of the disease.</p></sec><sec><title>Conclusion</title><p>Conclusion.
