Введение. В настоящее время наряду с совершенствованием схемы комбинированной медикаментозной терапии противовоспалительными препаратами первой линии важное значение придается адъювантным топическим методам лечения заболеваний дистальных отделов толстой кишки различного воспалительного генеза.

Цель

Цель. Оценить клиническую эффективность применения препарата на основе гиалуроната натрия в качестве адъювантного топического средства в комплексном лечении пациентов с заболеваниями дистальных отделов толстой кишки различного воспалительного генеза в амбулаторных условиях.

Материалы и методы

Материалы и методы. Проведен сравнительный анализ результатов медикаментозного лечения 97 пациентов с заболеваниями дистального отдела толстого кишечника различного воспалительного генеза (хронический язвенный ректосигмоидит и хронический лучевой проктосигмоидит), которые были разделены на 2 группы. У 46 (47,4%) пациентов I (контрольной) группы проводилась общепринятая (стандартная) медикаментозная терапия базисными противовоспалительными препаратами. У 51 (52,6%) пациента II (основной) группы стандартная программа медикаментозного лечения дополнялась инстилляциями в прямую кишку препарата на основе гиалуронат натрия, содержащего в 1 мл 0,8 мг гиалуроновой кислоты. При сравнительной оценке результатов лечения больных учитывали выраженность клинических, лабораторных и эндоскопических проявлений заболевания, а также характер морфологических изменений слизистой оболочки толстой кишки на материале эндоскопических колонобиоптатов в динамике лечения.

Результаты

Результаты. У пациентов основной группы отмечалось существенное сокращение сроков заживления эрозий и изъязвлений слизистой оболочки дистальных отделов толстой кишки по сравнению с больными в контрольной группе (соответственно 29,4 ± 5,7 против 52,8 ± 6,2 сут.). Также у пациентов основной группы удавалось в более ранние сроки от начала лечения достичь устойчивой клиниколабораторной ремиссии заболевания (включая редукцию болевого синдрома, купирование кишечных симптомов и проявлений интоксикации, нормализацию уровня фекального кальпротектина), восстановления трудоспособности пациентов, а также повышения качества их жизни, чем в контрольной группе больных. Это позволило увеличить число хороших и удовлетворительных результатов лечения у данной категории больных с 76,1 до 88,2%.

Заключение

Заключение. Полученные нами результаты позволяют рекомендовать препарат на основе гиалуронат натрия в качестве дополнительного топического лекарственного средства при проведении медикаментозного лечения хронических заболеваний дистальных отделов толстой кишки различного воспалительного генеза в амбулаторных условиях.

Introduction

Introduction. Currently, along with the improvement of the scheme of combined drug therapy with first-line anti-inflammatory drugs, important importance is attached to adjuvant topical methods of treating diseases of the distal colon of various inflammatory genesis.

Aim

Aim. Evaluate the clinical efficacy of the drug on sodium hyaluronate as an adjuvant topical agent in the complex treatment of patients with diseases of the distal colon.

Materials and methods

Materials and methods. A comparative analysis of the results of drug treatment of 97 patients with diseases of the distal colon of various inflammatory genesis, which were divided into two groups, was carried out. In 46 (47.4%) patients of the I (control) group, standard drug therapy with basic anti-inflammatory drugs was performed; 51 (52.6%) patients – II (main) group, the standard program of drug treatment was supplemented with instillations into the rectum of the drug on sodium hyaluronate (0.8 mg of hyaluronic acid in 1 ml). The comparative assessment of the results of treatment took into account the severity of clinical, laboratory and endoscopic manifestations of the disease.

Results

Results. In patients of the main group, there was a significant reduction in the healing time of erosions and ulceration of the mucous membrane of the distal colon compared with patients in the control group (29.4 ± 5.7 days, respectively, versus 52.8 ± 6.2 days). Also, in patients of the main group, it was possible to achieve stable clinical and laboratory remission of the disease at an earlier time, restoration of patients’ ability to work, and improvement of their quality of life.

Conclusion

Conclusion. The results obtained by us allow us to recommend the drug on sodium hyaluronate as an additional topical drug for the medical treatment of chronic diseases of the distal colon of various inflammatory genesis in outpatient settings.
