По мере развития флебологии и широкого внедрения ультразвуковых методов появилась и хорошо зарекомендовала себя как более эффективная техника микропенная склеротерапия. В мировой литературе есть единичные публикации, в которых говорится об эффективности и безопасности комбинации микропенной склеротерапии с мини-флебэктомией, но конкретных исследований, сравниваю щих комбинированное пособие по устранению варикозного синдрома с изолированным использованием каждой из методик, не описано. Цель. Повысить эффективность оперативного лечения и снизить частоту осложнений у пациентов с варикозной болезнью с использованием комбинации мини-флебэктомии и микропенной склеротерапии после эндовенозной лазерной коагуляции магистральных вен.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. На базе ГБУЗ «ГКБ им. А.К. Ерамишанцева ДЗМ» проведено одноцентровое простое нерандомизированное ретроспективное исследование. В него вошло 52 пациента (77 нижних конечностей; двустороннее поражение выявлено у 22 пациентов) с диагнозом «варикозное расширение вен нижних конечностей», которые не проходили хирургическое лечение по данному заболеванию в прошлом и которым была выполнена эндовенозная лазерная коагуляция в сочетании с микропенной склеротерапией и мини-флебэктомией. Микропенную склеротерапию выполняли пенной формой 0,5–2% раствора полидоканола, а мини-флебэктомию по методу Варади. В послеоперационном периоде пациентов наблюдали клинически и с помощью дуплексного сканирования на следующий день после вмешательства, далее через 1, 6 и 12 мес.</p></sec><sec><title>Результаты</title><p>Результаты. Облитерация магистральных вен была успешна во всех случаях. Реканализация больших подкожных вен за период наблюдения не зарегистрирована. В ближайшем послеоперационном периоде комбинация микропенной склеротерапии и мини-флебэктомии продемонстрировала 100% успех, однако в отсроченном периоде в 4 (5,2%) случаях были выявлены рецидивные варикозно расширенные притоки, которые потребовали повторной микропенной склеротерапии.</p></sec><sec><title>Выводы</title><p>Выводы. Микропенная склеротерапия в сочетании с последующей мини-флебэктомией как метод устранения варикозного синдрома при лечении пациентов с варикозной болезнью вен нижних конечностей может обеспечить дополнительные преимущества в виде уменьшения объема и травматичности мини-флебэктомии, частоты рецидива варикозного синдрома, количества подкожных гематом и экхимозов, дискомфорта при выполнении большого количества тумесцентной анестезии, риска развития послеоперационного тромбоза притоков и частоты возникновения пигментации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The development of phlebology and mainstreaming of ultrasonic techniques has led to the emergence of microfoam sclerotherapy, which proved itself as a more effective technique. In the world literature, there are isolated publications that say about the effectiveness and safety of the microfoam sclerotherapy combined with mini-phlebectomy, but no specific studies comparing the combination treatment for the elimination of varicose syndrome with separate use of each of the techniques are described.</p></sec><sec><title>Aim</title><p>Aim. To increase the effectiveness of invasive treatment and to reduce the rate of complications in patients with varicose veins using a combination of microfoam sclerotherapy and mini-phlebectomy of tributaries after endovenous laser ablation.</p></sec><sec><title>Material and methods</title><p>Material and methods. Simple single- center, non-randomized, retrospective study was conducted at the A.K. Eramishantsev Moscow State Hospital. It included 52 patients with varicose veins (a total of 77  lower  limbs, 22 patients had bilateral disease), who had no previous invasive treatment for this disease. They underwent endovenous laser ablation of truncal vein combined with microfoam sclerotherapy and mini-phlebectomy of tributaries. Microfoam sclerotherapy was performed with 0.5–2.0% of polidocanol foam, and mini-phlebectomy per Varady technique. The patients had a postprocedural follow-up clinical examination and duplex ultrasound the day after the intervention, then at 1, 6, and 12 months.</p></sec><sec><title>Results</title><p>Results. Endovenous laser ablation of truncal veins was acutely successful in all cases. No cases of great saphenous vein recanalization were detected in follow-up period. In the early postprocedural period, the combined microfoam sclerotherapy and miniphlebectomy also showed 100% success rate, however redo sclerotherapy was required in 4 (5.2%) cases for new varicose tributaries developed in late postprocedural period.</p></sec><sec><title>Conclusion</title><p>Conclusion.
