Варикоцеле – варикозное расширение вен яичка, вызванное патологическим венозным рефлюксом – наиболее распространено у бесплодных мужчин. Клинически варикоцеле обнаруживается в мужской популяции у 15%, при этом у 35% мужчин с первичным бесплодием и у 70–81% мужчин с вторичным. Варикоцеле, не сопровождаемое болевым синдромом, является наиболее частой причиной развития патоспермии и бесплодия. Оно выявляется у 1% мальчиков в возрасте до 10 лет, к периоду полового созревания достигая значения в 15%. В возрасте 50–80 лет частота возникновения варикоцеле увеличивается до 75–77,3%, таким образом прибавляя в среднем по 10% каждые 10 лет жизни.</p><p>Цель – исследовать восстановление фертильности после хирургического лечения и прогностические критерии ее восстановления в зависимости от возраста и предлагаемой медицинской реабилитации.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В урологическом стационаре краткосрочного пребывания «Краевой клинический специализированный уроандрологический центр» (Ставрополь) в 2011–2019 гг. было проведено 1143 варикоцелэктомии (244 лапароскопические варикоцелэктомии, 975 микрохирургических варикоцелэктомий). В основе исследования лежит сравнительный анализ результатов обследования 100 пациентов с патоспермией и варикоцеле в период с 2011 по 2019 г., получивших хирургическое лечение и не имевших сопутствующей патологии. Все пациенты после операции наблюдались в течение 18 мес.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Болевой синдром купирован у 100% исследуемых пациентов. Прогрессии гипотрофии левого яичка в послеоперационном периоде не выявлено. Рецидивов и других осложнений не отмечено. Пациенты контрольной группы также в течение 1,5 лет наблюдались с интервалом 6 мес. Ухудшения показателей сперматогенеза в данной группе не выявлено.</p></sec><sec><title>Выводы</title><p>Выводы. Выявлено, что варикоцеле явилось основной причиной нарушений сперматогенеза и фертильности в исследуемых группах. Патогенетически обоснованным и статистически доказанным методом восстановления фертильности при варикоцеле является оперативное лечение. После варикоцелэктомии адекватная медикаментозная реабилитация позволяет нормализовать качественные и количественные показатели спермы в большинстве клинических случаев в кратчайшие сроки (до 3 мес.).</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Varicocele is a testicular varicose vein caused by pathological venous reflux. It is most common in infertile men. Clinically, varicocele is found in 15% of the male population, while in 35% of men with primary infertility and in 70–81% of men with secondary infertility. Varicocele, not accompanied by pain, is the most common cause of the development of pathospermia and infertility. Varicocele is detected at the age of 10 years in 1% of boys, increasing to puberty up to 15%. At the age of 50–80 years, the incidence of varicocele increases to 75–77.3%, thus adding an average of 10% every 10 years of life.</p><p>Aim is to investigate the fertility restoration after surgical procedures and the prognostic criteria for its restoration depending on age and the proposed medical rehabilitation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. In the urological hospital for a short stay of the state autonomous healthcare institution of the Stavropol Territory “Regional Clinical Specialized Uro-Andrology Center” from 2011–2019. 1143 varicocelectomies were performed (244 laparoscopic varicocelectomy, 975 microsurgical varicocelectomy). The study is based on a comparative analysis of the results of examination of 100 patients with pathospermia and varicocele in the period from 2011 to 2019, who received surgical treatment and did not have concomitant pathology. All patients were followed up for 18 months after surgery.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The pain syndrome was stopped in 100% of cases in the studied patients. Progression of hypotrophy of the left testicle in the postoperative period was not detected. Relapses and other complications were not noted. Patients in the control group were also observed for 1.5 years with an interval of 6 months. Deterioration of spermatogenesis in this group was not detected.</p></sec><sec><title>Conclusions</title><p>Conclusions. It was revealed that varicocele was the main cause of spermatogenesis and fertility disorders in the studied groups. Surgical treatment is a pathogenetically grounded and statistically proven method of restoring fertility in varicocele.
